People with Disabilities
If you are a person with a disability, you are not alone. About 1 in 4 adults in the United States reports having some form of a disability. Each of us is unique, and you likely know your needs and preferences best. You may be looking for ways to take better care of your health, find a better place to live or help to work with Social Security and other government agencies. We know the public benefits are complex and it can be overwhelming to navigate these systems. We can help build your network of support in the ways you need and desire. Our staff of experienced service coordinators will ensure you feel safe and protected and your quality of life is the best it can be. OUR MISSION
PLAN|NJ’s mission is to help family members of people with disabilities answer the question:
“Who will care for my loved one when I'm gone?”
We coordinate the supports needed by individuals with intellectual, developmental and physical disabilities, and challenges with mental health, including people with autism, cerebral palsy, traumatic brain injury, severe mental illness and more.
