Human Service Providers

Home 9 Circle of Support: Human Service Providers

At PLAN|NJ, we provide compassionate care coordination services for people with disabilities and their families. As human service providers, you are an essential part of the wide variety of support they need. While serving your fellow human beings, you help them alleviate stress and manage change, and encourage them to function at their highest capacity.

Whether you are a residential service provider, a direct support professional, a medical service provider such as a nurse, a child welfare caseworker, a psychologist or Licensed Clinical Social Worker, or if you work in another human services profession, you empower people to thrive and enable them to enjoy life as a fully participating member of the community.

OUR MISSION

PLAN|NJ’s mission is to help family members of people with disabilities answer the question:

“Who will care for my loved one when I'm gone?”

We coordinate the supports needed by individuals with intellectual, developmental and physical disabilities, and challenges with mental health, including people with autism, cerebral palsy, traumatic brain injury, severe mental illness and more.
