CLIENT STORY – DANIEL 

Daniel and his mother enjoy travel, and have visited Tokyo, Las Vegas, Beijing, Hong Kong, and Argentina.  Photos are added to his LifePLAN to help provide a complete picture of his quality of life. Health and strength-building activities are on his regular schedule, such as training for the Special Olympics.

He has participated in physical therapy using a horse for over 17 years. In addition, Daniel is an ongoing member of an adaptive ski and sports program in Vermont. He regularly attends music therapy, and has participated in choir concerts using his lefthand switch. When prompted by the conductor, Daniel hits the switch, which elicits a sound through a music synthesizer. “I locate and provide appropriate activities that I feel will excite Daniel, and I take advantage of my good health. We hope to inspire others who may feel that a person living with significant disabilities is restricted in what they can do,” said his mother.

The stories of three families with loved ones who have disabilities and who use PLAN|NJ's trust administration and care coordination services are told through interviews conducted in their homes by Executive Director Ellen Nalven.

OUR MISSION

PLAN|NJ’s mission is to help family members of people with disabilities answer the question:

“Who will care for my loved one when I'm gone?”

We coordinate the supports needed by individuals with intellectual, developmental and physical disabilities, and challenges with mental health, including people with autism, cerebral palsy, traumatic brain injury, severe mental illness and more.
